Do I need a referral?

  • General - A referral is not necessary. However, if you have a list of your medical history, medications and most recent blood tests from your GP that would be helpful for your assessment.

  • Medicare- Please bring the GP Chronic Disease Management Plan referral.

  • Department of Veterans’ Affairs- Please bring the GP DVA referral.

  • National Disability Insurance Scheme- Please bring your NDIS participant’s plan.

  • Home Care Package- Please bring your My Aged Care support plan or Home Care Package provider referral.

  • Work Cover- Please bring your approved treatment plan. 

Can I claim anything back from nutrition consultations?

  • General- Yes, if you have dietetics cover. Please check with your health insurance. A receipt will be provided for you to claim from your insurance. 

  • Medicare- With a GP Chronic Disease Management Plan you can claim $58.30 for each consultation up to 5 times yearly. This rebate can be processed for you, you will receive a receipt and the rebate will go to your bank account linked to Medicare  (https://www.humanservices.gov.au/individuals/enablers/submit-medicare-claim-online). Bulk-billing with nil out-of-pocket expenses is available for 30 minute sessions if you have a valid pension or low income health care card up to practitioner discretion (put this in the notes section when booking).

  • Department of Veterans’ Affairs- Nil out-of-pocket expenses. Payment will be claimed from DVA.

  • National Disability Insurance Scheme- Nil out-of-pocket expenses. Payment will be claimed from allocated NDIS funds as per NDIS Price Guide.

  • Home Care Package- Payment will be claimed from your Home Care Package provider.

  • Work Cover- Payment will be claimed from your insurer.

How do I make payment?

Pay via direct deposit transfer without processing fees. For card or PayPal payments, processing surcharges of $1.00 + 1.90% apply. For Work Cover, Home Care Packages, or NDIS, invoices will be sent directly to your insurer or provider.  

What does the initial consultation involve?

1. Understanding your needs: This is where we discuss what you would like to gain from the  consultation and your overall health goals.

 

2. Assessment: There will be a detailed assessment that goes through your medical history, family medical history if relevant, medications and supplements that you take if any, any allergies or intolerances, your exercise and activity habits, reported weight and waist circumference measurements if you are comfortable and have them available (totally up to you!), any blood and urine test results you have available which could help you track improvements, clinical factors that may be relevant such as blood pressure, energy levels, appetite, bowel and sleep habits, followed by who does the cooking and shopping, how often, and going through your usual dietary intake including types and amounts. 

 

3. Recommendations, planning and goal setting: This involves going through your nutritional requirements, any relevant resources, discussing what could help you achieve your overall health goal, making a plan and setting realistic initial day-to-day goals together with you so you can implement practically and immediately after the consultation.   

What do review consultations involve?

1. Supporting You: We will go through how you went with implementing the initial day-to-day goals, keeping you accountable, discuss and overcome any barriers you may face. We can make changes to the initial day-to-day goals if appropriate. A lot of the time you select 1-3 goals to start with and when they have become a habit and you feel comfortable, we could go through any relevant resources and plan new day-to-day goals to help you further along towards achieving your overall health goal.

Even if you made zero changes, follow-up is critical for success. We will discuss your thoughts, feelings, situations, and strategies on how you can make healthy eating part of your lifestyle. The most important part is making sure these habits are practical for you in the long term for sustained life changes. Changing habits takes time and planningI will be here to support you all along the ups and downs of your health journey!

2. Evaluation and Assessment: We will assess the effectiveness of the developed strategies and compare this to those reported in the previous consultation(s) if relevant and appropriate such as weight and waist circumference (totally up to you!), blood test results such as total, LDL and HDL cholesterol, triglycerides, blood sugar, insulin, glycated haemoglobin A1c (average blood sugar indicator that changes approximately every 2-3 months), liver function, kidney function, blood pressure, energy levels, appetite and bowel habits, etc. 

How frequent are review consultations and how many are required ?

This totally depends on you, your needs and health goals.

  • When starting to make new lifestyle changes, more frequent review consultations are generally recommended so you will have the support you need and have any questions you have answered such as every 1-2 weeks.

  • As you progress, the changes are becoming part of your lifestyle, you become more confident and comfortable, you could gradually reduce the frequency to 3-4 weeks.

  • When the habits have become part of your lifestyle, depending on your comfort and needs, the frequency could be reduced to monthly, bi-monthly, followed by every 3-6 months to continue addressing any concerns, tracking improvements, and providing support.

  • When you are satisfied you have achieved your health goals and confident you can maintain the healthy eating as part of your lifestyle in the long-term, you can choose to have an annual review or stop the 1-On-1 consultations. What is your cancellation policy?

Our dietitian allocates up to 60 minutes for each appointment and we do not double book appointment slots. Clients are therefore kindly asked to keep appointments, unless an emergency occurs. Cancellations or postponements within 48 hours of appointment time will attract a full consult fee.
